Client Service Contract

I understand and agree to all of the following:

  1. No sexual activity is permitted. If a client tries to force themselves sexually onto an Alt Touch Specialist, the Alt Touch Specialist has the right to leave.

  2. Both parties will remain clothed the entire session. If either party needs to change clothing this will be done in private and out of sight of the other party. If a client shows off areas normally covered by undergarments purposely, the Alt Touch Specialist has the right to leave. If the client has a clothing request that is season appropriate, you may ask (example: an Alt Touch Specialist being asked to wear a red sweater in winter instead of a blue long sleeve shirt).

  3. No touching in areas covered by undergarments is permitted. If this occurs, the Alt Touch Specialist has the right to leave. No kissing is allowed.

  4. Phone contact between Alt Touch Specialists and client is to schedule appointments ONLY.

  5. All appointments must go through Alternative Touch Therapy LLC. For every new session, it must be scheduled through the website, a call or e-mail must be made informing Alternative Touch Therapy.

  6. Alternative Touch Therapy LLC knows all appointment dates, times, and locations. Alt Touch Specialist will check in after appointments to confirm they left safely.

  7. The Alt Touch Specialist will not break any federal, city, state or county laws, statutes or regulations while performing services with the client.

  8. The client will not break any federal, city, state or county laws, statutes or regulations while with the Alt Touch Specialist.

  9. If an Alt Touch Specialists safety is at risk, a Alt Touch Specialist may carry a non-lethal personal defense item to defend themselves.

  10. Payment for each session is due before the session or at the very start of one.

  11. If you cancel within 24 hours of a session or don’t show up and want another session in the future, you must prepay for the next session online AND pay a $40 cancellation fee which may be charged to your credit card on file. If you RESCHEDULE within 24 hours of a session, there is a $20 rescheduling fee that may also be charged to your card on file.

  12. Cleanliness and adequate hygiene is required by both parties. This specifically includes showering/teeth brushed within 2 hours of the appointment, wearing freshly laundered clothing, and no (or minimal) perfume/cologne be worn by both parties unless requested.

  13. The client agrees hereby to indemnify and hold harmless Alternative Touch Therapy LLC from any and all claims by the client which may arise out of and in the course of the service. The client releases The Alternative Touch Therapy LLC from any liability.

  14. An Alt Touch Specialist may end a session at any point for any reason. If the session has already began, a full or partial refund may be issued at the discretion of the company.

  15. Sessions may be recorded with both audio and video.

Release of Liability

I under the terms of this Agreement and sufficiency of which is hereby acknowledged, do hereby release and forever discharge Alternative Touch Therapy LLC including their agents, employees, successors and assigns, and their respective heirs, personal representatives, affiliates, successors and assigns, and any and all persons, firms or corporations liable or who might be claimed to be liable, whether or not herein named, none of whom admit any liability to the undersigned, but all expressly denying liability, from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever, which I now have or may hereafter have, arising out of or in any way relating to any and all injuries and damages of any and every kind, to both person and property, and also any and all injuries and damages that may develop in the future, as a result.

It is understood and agreed that this Agreement is made and received in full and complete settlement and satisfaction the causes of action, claims and demands mentioned herein; that this Release contains the entire Agreement between the parties; and that the terms of this Agreement are contractual and not merely a recital. Furthermore, this Release shall be binding upon the undersigned, and his respective heirs, executors, administrators, personal representatives, successors and assigns. This Release shall be subject to and governed by the laws of the State of Idaho.

This Release has been read and fully understood by me.

BY SCHEDULING A SESSION, YOU ARE AGREEING TO THE CLIENT CONTRACT AND WAIVER AND RELEASE ALL LIABLITY
